What makes care for Veterans different
Military life shapes how people drink, cope and ask for help. A Veteran may have learned to push through pain and keep problems quiet. Some started drinking heavily in the service, where it could feel like part of the culture. Others started after coming home, when the structure and the team were gone.
The VA lists posttraumatic stress disorder (PTSD) and depression among the conditions that can be related to substance use problems, and it treats them alongside substance use. If your loved one has trauma symptoms, ask any program how it treats both at once. Our page on dual diagnosis treatment explains why that matters.
Many Veterans also open up more with other Veterans. The VA says many Veterans Crisis Line responders are Veterans themselves. When you call a program, ask whether it has Veteran groups or staff who served.
What the VA offers for substance use
According to the VA’s substance use treatment page, VA care for substance use problems can include:
- Medically managed detox and services to get stable
- Medicines to reduce cravings, like methadone and buprenorphine for opioid addiction
- Short-term outpatient counseling and intensive outpatient treatment
- Residential (live-in) care
- Marriage and family counseling, self-help groups and relapse prevention
- Special programs for Veterans with specific concerns, like women Veterans, returning combat Veterans and Veterans who are homeless
The VA says to apply for VA health care first. A Veteran with a VA primary care provider can talk to that provider about substance use. One without a VA provider can find a substance use program on VA.gov, call the VA’s general information line at 800-827-1000 or contact the local VA medical center.
If they don’t have VA health care
- Served in a combat zone? The VA says community Vet Centers offer free private counseling, alcohol and drug assessment and other support.
- Homeless or at risk? The VA’s National Call Center for Homeless Veterans is open 24/7 at 877-424-3838.

Which level of care often fits
| Often a fit when | Ask about | |
|---|---|---|
| Detox | Heavy daily drinking, opioid use or sedative use, where withdrawal could be dangerous | Whether the VA or the program can manage withdrawal medically |
| Residential | Use is severe, home isn’t stable or outpatient care hasn’t worked | Trauma care alongside substance use treatment |
| Intensive outpatient (IOP) | The Veteran can live at home and needs several sessions a week | Evening groups if they work, and Veteran-only groups |
| Outpatient and aftercare | After a higher level of care, or for early problems | Peer support and how the plan continues after discharge |

Questions from Veteran families
Can a Veteran get VA substance use care without being enrolled?
To use VA substance use services, the VA says to apply for VA health care first. In a crisis, the Veterans Crisis Line helps any Veteran, enrolled or not. Combat Veterans may also get free counseling and assessment at a Vet Center.
What if my Veteran refuses help?
That is common, and pushing hard often backfires. Try a calm talk when they are sober, and consider family support for yourself. Our guide on how to talk to a loved one can help. If they are in danger, call 911 or dial 988 then press 1.
Does the VA help families too?
The VA lists marriage and family counseling among its substance use services. The Veterans Crisis Line also serves families and friends who are worried about a Veteran.
Can a Veteran use a private program in Jacksonville?
Sometimes, depending on their coverage and VA rules for community care. Ask the VA before admission so there are no surprise bills. Our VA and TRICARE guide has the questions to ask.
Is PTSD treated at the same time as substance use?
It can be. The VA treats PTSD and depression as related conditions, and many programs offer dual diagnosis care. Ask each program how it handles trauma during treatment.
